Who You Are You are a proactive individual with a positive attitude and a can-do spirit. You possess a Social Care qualification, such as NVQ/QCF Level 3 in Adult Social Care. You have at least 2 years of experience supporting vulnerable adults to achieve their reablement goals. You are competent in using IT systems and maintaining electronic records, including Excel and Word. You hold a clean driving license and have access to a vehicle for travel. You demonstrate appropriate behaviours such as accountability, respect, and collaboration in all dealings. You are able to communicate clearly and effectively in spoken and written English. What the Job Involves Providing high-quality support to service users to help them plan and achieve care and health outcomes. Contributing to service delivery in accordance with local policies and legislation. Working closely with a therapy team to implement reablement plans, including prescribing necessary equipment. Assisting in the induction and training of new staff members. Establishing and sustaining effective relationships with service users and supporting their inclusion and participation. Ensuring service users have access to advocacy and supporting them in self-advocacy where appropriate. Actively participating in team workshops and learning opportunities to promote good practice. Collaborating with health professionals, carers, and voluntary organisations to support users and families. Complying with the Council’s policies, including Health & Safety, Data Management, and Information Security. Undertaking tasks flexible to varying hours and working conditions as required for effective service delivery.
