The role:
The role provides an exciting opportunity to work in a multi-disciplinary setting, with a focus on strengths-based practice and promoting independence and recovery.
The main duties of the role include:
1. Goal setting
2. Completing Care Act assessments
3. Devising care and support plans
4. Completing reviews
5. Completing Mental Capacity Assessments
6. Chairing Best Interests meetings
7. Crisis intervention
8. Signposting
9. Providing advice and information
10. Working with people with complex social needs
There is also a Safeguarding function within the service, which includes completing initial fact-finding and assessing and minimising risk.

Our Intermediate Care Service (ICS) is seeking Social Workers to work across both integrated teams, Community Urgent Response and Reablement. The role offers a unique opportunity to thrive in a multi-disciplinary environment, collaborating closely with health professionals. We place a strong emphasis on strengths-based practice, crisis management, independence, and recovery, ensuring that each individual receives comprehensive support tailored to their needs.
The role encompasses a diverse range of tasks, including:
* Providing advice and information to service users and their families
* Goal-setting for reablement and rehabilitation
* Conducting Care Act assessments and person-centred care and support plans
* Facilitating urgent reviews and Mental Capacity Assessments + Best Interests meetings
* Delivering crisis intervention and timely signposting to necessary community resources
* Working closely with individuals with varied health and social care needs, including informal carers
Within our service, safeguarding is paramount. The role also includes conducting initial fact-finding, risk assessment, and mitigation to ensure the safety and well-being of vulnerable individuals.
This post requires a satisfactory Disclosure and Barring Service (DBS) check at Enhanced level.
