The successful candidate will play a key role in proactively identifying and supporting patients both after diagnosis (such as delivering Cancer Care Reviews) and before diagnosis (supporting the uptake of the national cancer screening programmes). They will work closely with GPs and other primary care colleagues within the Primary Care Network (PCN) and working to improve the integration between Primary and Secondary Care. The successful candidate will have excellent interpersonal and communication skills and be organised, patient and empathetic. They will have experience of working in health, social care or other support roles, including direct contact with people, families or carers.
