Practice Plus Group, Emersons Green and Devizes are hiring a part-time (22.5 hrs per week) Clinical Coder who will be based at our Emersons Green Hospital in Bristol. This is an exciting opportunity to grow and develop your career in Clinical Coding, under the supervision and guidance of our Senior Finance Manager. You will apply your experience of having previously worked in Healthcare administration and patient administration systems.
About the Role:
To analyse, interpret, abstract relevant information (including complex conditions and treatments) and code patients clinical case notes using Nationally Agreed Classifications and Codes currently ICD10 and OPCS 4 classifications as per Trust requirements. To ensure accurate, timely and consistent clinical coding within set national standards and targets. (This coded information is then used in Payment by Results; analytical and epidemiological studies, and for planning and information both locally and nationally.)
Main Duties of the Job
The Clinical Coding Department is responsible for ensuring that all patient activity captured on Practice Plus Groups Patient Administration System (Maxims) is accurately clinically coded within the set national timeframes for charging Clinical Commissioning Groups (CCGs). * Evidence of further study. NACC accredited or working towards this currently.

Job Responsibilities
Key Objectives of the Post Principal Duties and Responsibilities
To code diagnosis and clinical procedures of all inpatient and day-case activity using ICD-10 and OPCS-4 classifications and local policies and procedures, by analysing, abstracting and interpreting all complex clinical data in accordance with national and international rules and conventions and to input the coded information onto the Patient Administration System (PAS) in a timely and accurate manner.
* To undertake ongoing assessment of work to ensure continuous improvement in both the assessment of medical record content and accuracy of coding.
* To work under supervision whilst developing ability to work on own initiative.
* To acquire, through work-based training, a knowledge of coding procedures and conventions and medical terminology, anatomical and physiological terms.
* To ensure adherence to policies and procedures established for coding, referring more complex coding and data quality issues to senior coding staff.
* To become proficient in recognising inconsistent, ambiguous or non-specific information in either the medical record or held on the PAS and to liaise with medical staff to resolve any queries relating to clinical information.
* To liaise with Clinicians, Medical Records personnel and admin and ward personnel as appropriate.
* To become competent in the use of and establish a working knowledge of relevant elements of internal systems and NHS Trust pathology systems.
* To locate and collate missing source material (eg. histology reports, op notes, referrals etc.)
* To participate in regular clinical coding audits in order to maintain high clinical coding standards.
* To take responsibility for prioritising and managing workload.
* To complete the coding of all finished consultant episodes within 5 days of discharge date.
* To participate in relevant training sessions and coding courses to ensure compliance with national coding guidelines.
* To understand and comply with the Clinical Coding Policy and Procedure document of Practice Plus Group.
* To take responsibility for ensuring patient information is always kept confidential and only used for the purpose required.
* To deal with the daily exposure of sometimes disturbing photographic material and information in a patients medical record.
* To undertake any other tasks as requested by the ACC Clinical Coder, or Finance Manager.
* To be responsible for complying with Emersons Green Infection Control policies and clinical guidelines.
* To prepare and actively participate in Practice Plus Groups annual Performance Conversation Review process.
* To ensure training and development needs detailed in the performance conversation review are followed up and the effectiveness of the acquired training and development evaluated in terms of self, service and patient.
* To be responsible for the quality of data recorded. The data should be accurate, legible (if handwritten), recorded in a timely manner, kept up to date and appropriately filed.
