The Norwich Triage team currently has a vacancy for a part-time Community Triage Nurse.
The Triage Hub is the starting point of the patient journey into community services, we take great pride in our work and always aim to do the best for our patients. We are looking for someone who holds the same values and wants to be part of a diverse and supportive team.
We are a small and friendly team of nurses who triage patient referrals from across the Norwich area, assessing the patient's needs and determining the priority of care. The Community Triage Nurse is a pivotal role within the Triage Hub and is essential in ensuring patient flow across the service.
Main duties of the job
You should be competent and have evidence-based knowledge of a wide array of nursing interventions to be able to triage effectively. Supporting and coaching patients via telephone is also part of the role, therefore proficient telephone and IT skills are important. Although experience of Triage would be an advantage, it is not essential, as full training will be provided. The role allows for development of enhanced assessment skills with clinical judgement being at the forefront.
Job responsibilities
* To process referrals by evaluating the information received and allocating to the appropriate service indicating the priority response necessary.
* To document triage notes in order that the patients record is complete and for audit purposes.
* To use information technology to process the referrals for the 80% triaging role.
* To liaise and communicate with the general public, other health care professionals, and multi-agencies on a daily basis.
* To assess, prescribe and deliver care packages for patients, for example with long term conditions, palliative care and rehabilitation needs, to achieve quality of life and independence where possible.
* To work within the integrated team to facilitate early discharge from hospital.
* To work within the integrated team to prevent unnecessary admission to hospital.
* To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our service users.

Other Requirements
* Must hold full UK driving licence and have access to a vehicle
* Able to make own travelling arrangements
* Be able to kneel, bend and stoop and work in cramped environments.
* This is a 7 day a week service and some weekend working will be expected. Hours of service delivery, 8am - 6:30pm
* The post holder will need to be passed fit to perform full duties of the post through occupational health with any reasonable aids and adaptations if necessary.
* Able to manoeuvre limbs of around 5kg - 6kg
* Able to manoeuvre patients using handling aids
