Job summary Care Navigator/Data Administrator Forum Family Practice We are looking to recruit a friendly, motivated, enthusiastic, and ideally experienced Care Navigator/Data Administrator to join our supportive team. In this role, youll play a crucial part in helping our patients, both face-to-face and over the phone, with their healthcare needs. Youll be a key part of ensuring patients receive the right help at the right time. Ideally, youll have previous experience working in a GP practice or within the NHS, and some knowledge of SystmOne though this is not essential, as training will be provided. If you enjoy variety in your work, have strong IT skills, and pride yourself on delivering excellent customer service, this could be the perfect role for you. We are looking for someone to work a minimum of 25.5 hours per week (MondayWednesday). Main duties of the job Main Duties: Support the clinical team by signposting patients to the appropriate healthcare professional or service, working as part of the multidisciplinary team. Process patient appointment requests, both in person and over the phone. Answer incoming calls, transferring them or dealing with enquiries appropriately and efficiently. Initiate contact with, and respond to, requests from patients, colleagues, and external agencies in a timely and professional manner.
