Job Title: Qualified Social Worker – Hospital Discharge Team
Location: Lancashire
Rate: £37 per hour
Liquid Personnel is currently recruiting for a Social Worker to join a busy Hospital Discharge Team based in Lancashire.
What will your responsibilities be?
You will undertake section 2s and 5s, facilitate hospital discharges, and carry out assessments in different wards. You will manage a busy caseload within an acute hospital setting and work closely with health colleagues. You will ensure adults in hospital are discharged in a timely and appropriate manner, attend multi-disciplinary meetings, and conduct mental capacity assessments leading to best interest decisions. Additionally, you will advise patients and families who do not meet the eligibility criteria and redirect them to other agencies.

Qualifications and Experiences:
You must have Social Work England registration and be eligible to work in the UK. Additionally, you need to hold a full UK driving licence and possess post-qualified experience. A degree or equivalent qualification in Social Work is also required.
