Duties:
* Undertake comprehensive assessments to assess the eligibility and suitability, strengths and needs of a service user.
* Devise, monitor, review and update recovery care plans to ensure service users’ needs are met in a timely manner.
* Deliver care packages including psychosocial and psychological interventions aimed at engagement.
* Conduct risk/need assessments and implement risk management/care plans in line with policy.
* Liaise with other health and social care professionals as necessary.
* Function as part of a multi-disciplinary team, contributing to team meetings.
* Case-manage in line with a harm reduction & recovery focused model.
* Communicate sensitive information within an often hostile and emotive environment.
* Assist in the decision-making process regarding the referral of service user’s children to the relevant safeguarding agency.
* Compile reports and participate in case conferences and multi-disciplinary team meetings.
* Screen for alcohol use.
* Collate statistical information as required to meet the needs of key stakeholders.
Requirements for the role of Substance Misuse Case Manager:
* Recent experience in Substance Misuse, Substance Abuse, Addiction, Drug and Alcohol, Needle Exchange, or Recovery.
* Relevant qualifications or certifications.
