Job summary
We are seeking an experienced and highly skilled Senior Mental Health Practitioner to join the West Suffolk Crisis Resolution and Home Treatment Team (CRHTT). The successful candidate will play a pivotal role in providing high-quality, person-centred care to individuals experiencing acute mental health crises, ensuring that service users are treated in the least restrictive environment and supporting their recovery.
As a Senior Mental Health Practitioner, you will lead and supervise a dynamic, multidisciplinary team, providing clinical leadership and mentoring to junior staff. You will work closely with service users, families, and other healthcare professionals to deliver tailored care plans that promote recovery, while ensuring a seamless transition from acute crisis care to ongoing support.
Main duties of the job
Clinical Leadership: Take the lead in managing a dynamic and shared complex caseload, providing direct clinical assessments, and delivering evidence-based interventions to individuals in mental health crises. Ensure the delivery of high-quality, person-centred care within the home treatment model.
Care Coordination: Lead on coordinating care and ensuring effective treatment plans are in place. Work closely with multidisciplinary teams, including psychiatrists, social workers, psychologists, and other healthcare professionals, to deliver comprehensive and cohesive support for service users.
Risk Assessment and Management: Undertake risk assessments and develop risk management plans for individuals in acute mental health crises. Monitor service user progress and adapt care plans as necessary, ensuring patient safety is maintained at all times.
Patient Advocacy: Promote the voice and choice of service users, ensuring they are involved in their care plans and supported in making informed decisions. Work collaboratively with service users and their families to support recovery and resilience.
Service Improvement: Contribute to the development of services and clinical practices within the team.
Crisis Intervention: Provide immediate crisis assessment, intervention and home treatment to individuals in acute mental health distress, aiming to reduce hospital admissions and promote recovery in the community.

Qualifications Essential
* Nursing Degree or equivalent, Social Work Degree or equivalent, Dip OT
* Ongoing professional development working towards degree level
Experience Essential
* Demonstrate effective risk assessment and risk management experience
* Experience in the delivery of clinical interventions
* Experience in working in a team delivering person centred treatment packages
Skills Desirable
* Able to reflect on clinical practice
Knowledge Essential
* Knowledge of medication and the monitoring of its effects
* Knowledge of psychosocial interventions
