Job Description
The Role: The successful candidate will be supported by a Senior Occupational Therapist and an Assistant to manage a caseload of residents requiring assessment and intervention in preparation for hospital discharge. The role offers opportunities to build on knowledge and skills around hospital pathways and support team development.
Your Role Will Involve:
* Provide a professional Occupational Therapy / Reablement Service, including statutory Social Care responsibilities.
* Contribute to Service delivery that is person-centred and outcome-focused with an emphasis on encouraging independence and enabling recovery.
* Work collaboratively with other professionals and agencies to address complex needs and achieve best outcomes for individuals and their Carers.
* Work with health colleagues to ensure safe and timely discharges from hospital and step down beds in the community.
Person Specification
Qualifications Essential:
* Experience of working with vulnerable adults.
* Good communication and interpersonal skills, with the ability to engage with clients, families, and other professionals.
* Ability to work autonomously and manage competing priorities.
* Experienced in coordinating/liaising with multiple agencies/organisations.
* Implement evidence-based practice, keeping abreast of emerging research, best practices, and national frameworks (e.g., NICE guidelines).
Experience Essential:
* Experience of working with vulnerable adults in one or more of the following areas:
* - Learning Disabilities
* - Older Persons
* - Physical Disabilities
* - Mental Health
* - Experience of working with adults in the community.
* Experience working in any combination of adult social care, community settings, hospitals, or residential care environments.
