Main duties of the job
Role Overview:
You will work collaboratively with our multidisciplinary team to assess, plan, implement, and evaluate care, ensuring a patient-centered approach. This includes providing care to some frail and housebound patients.
As a senior member of our team, you will play a pivotal role in ensuring that nursing services are delivered efficiently and effectively to our patient population.
The Ideal Candidate:
We are looking for a candidate with expertise in chronic disease management, including experience in vaccinations, immunisations, and preventative healthcare. The successful candidate will be compassionate, proactive, and committed to delivering high-quality care in a collaborative, team-oriented environment.
Key Responsibilities:
* Chronic Disease Management: Oversee the care and management of patients with long-term conditions.
* Routine Nursing Care: Provide routine nursing care as needed, following clinical evidence, NICE guidelines, and National Service Frameworks (NSF).
* General Treatment Room Duties: Conduct a wide range of general treatment room duties, including vaccinations and immunisations, cervical screening, contraceptive reviews, wound care, and more.
* Clinical Policy Support: Lead on infection prevention and control, conducting audits, providing training, and ensuring compliance with IPC guidelines. You will also work closely with the practice management team to review and implement clinical policies.

Job responsibilities
Primary Key Responsibilities
The following are the core responsibilities of the Practice Nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:
1. To undertake the role of Infection Prevention Control (IPC) lead and to work to the IPC requirements by completing audits, training, support and evidence of compliance to the practice manager.
2. To develop, implement and embed an effective care management programme for the frail and housebound patients registered at the practice.
3. To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice.
4. To develop, implement and embed health promotion and wellbeing programmes.
5. To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing.
6. To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients' health.
7. As required, to provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF.
8. As required, to diagnose and manage acute and chronic conditions.
9. Provide wound care to patients.
10. To review patient medications to enhance compliance.
11. To provide guidance to patients in the use of prescribed and over-the-counter medications regarding side effects and dosages.
12. To co-ordinate the provision of travel medicine services.
13. To request pathology services and process pathology results as required.
14. To oversee the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required.
15. To maintain accurate clinical records in conjunction with extant legislation.
16. To ensure SNOMED CT codes are used effectively by all members of the nursing team.
17. To ensure the appropriate maintenance and use of chronic disease registers.
18. To ensure compliance with the practice chaperone policy.
19. To prioritise health issues and intervene appropriately.
20. To support the team in dealing with clinical emergencies.
21. To recognise, assess and refer patients presenting with mental health needs.
22. To implement vaccination programmes for adults and children.
23. To support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice).
24. To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.).
25. To delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual).
26. To support the clinical team with all safeguarding matters, in accordance with local and national policies, and understand practice/local policies for substance abuse and addictive behaviour, referring patients appropriately.
Secondary Key Responsibilities:
In addition to the primary key responsibilities, the Practice Nurse may be requested to:
1. Develop and review audit protocols and processes for the practice.
2. Develop, review and implement nursing protocols in conjunction with the partners.
3. Participate in local initiatives to enhance service delivery and patient care.
4. Support and participate in shared learning within the practice in order to improve patient care.
5. Continually review clinical practices, responding to national policies and initiatives where appropriate.
6. Participate in the review of significant and near-miss events applying a structured approach, i.e., root cause analysis (RCA).
7. Lead the planning and implementation of changes within the nursing team, enhancing the provision of services to patients.
8. Support the practice team with the reviewing and implementation of practice policies and protocols, ensuring conformance to extant legislation.

Qualifications

Experience
* Key Skills and Experience
* - Autonomous Practice: Experience working independently and making clinical decisions within scope.
* - Infection Control: Knowledge and application of infection prevention and control measures.
* - Chronic Disease Management: Experience with conditions like diabetes, hypertension, CHD, asthma.
* - Clinical Procedures: Skilled in wound care, ECGs, venepuncture, new patient medicals, immunisations, women's health (e.g., cervical cytology, contraception), pathology test requests, and travel medicine.
* - Evidence-Based Practice: Committed to applying evidence-based guidelines in patient care.
* - PGDs & Policies: Familiar with PGDs and relevant clinical policies.
