An exciting new opportunity for a qualified ACP to join our expanding team has arisen. We are seeking an enthusiastic ACP to join our Care Home Practice at St. James House.
An MSc in Advanced Practice is essential, and experience in General practice, frailty, end-of-life, Acute Medicine, ED, or other urgent care settings is desired.
The Care Home Practice is open from 0800 to 1900 Monday - Friday and 0800 to 1700 Saturday - Sunday to deliver services seven days a week. Although the working pattern is generally long days, we are open to negotiating shift times (where possible) to ensure a positive work-life balance.
Ability to travel across the area is essential.
You will be supported in clinical practice by our GP, clinical lead, and our ACP lead along with the wider team.
The Care Homes Medical Practice is a unique general practice that provides medical care to older people living in residential and nursing homes within Salford.
The Care Homes Medical Practice provides our registered patients with the same general medical services as any other general practice. However, we also provide additional services tailored to the special requirements of older people living in residential and nursing homes across the city of Salford.
The practice provides general medical services to their
registered patients but they also deliver additional proactive
care services tailored to the requirements of older people living in care homes, including:
• Weekly home rounds
• Detailed initial assessments and Personalised Care & Support Plans created for new patients
• Regular health checks and medicines reviews
• Services delivered by the Specialist Dementia nurse
• Support and Education from the Palliative Care Nurse team
• Clinical oversight from the lead GP to identify care home hot spots
The post holder will provide expert clinical and professional support within the team, demonstrating excellent interpersonal and communication skills and clinical attributes.
