A Vacancy at University Hospitals of Morecambe Bay NHS Foundation Trust.
The Post holder will act on behalf of the Head of Patient Safety and Complaints to investigate serious patient safety incidents occurring within the Trust, ensure effective support and engagement with affected persons including staff, patients, families and carers.
Identify learning from those investigations and support clinical and corporate services to make improvements in quality and safety.
Responsible for undertaking and leading the investigation process for a concurrent number of allocated serious patient safety incidents across the Trust, many of which will be characterised by high levels of complexity, sensitivity, contention and may contain distressing information.
To deputise for the Head of Patient Safety and Complaints as and when required
Oversee and support investigations of highly sensitive/complex matters and present findings in line with the Patient Safety Incident Response Policy.
Utilise a wide range of investigation techniques to analyse complex evidence and apply human factors/ergonomics analysis to incidents.
To monitor, review and report on changes which are implemented as part of the PSIRF work programme, ensuring that lessons are learnt and promoting a pro-active learning culture that encourages effective communication with all members of the multi disciplinary teams.
Prepare complex written investigation reports and advise the Director of Governance and Assurance, Associate Director of Governance, Chief Nursing Officer and Chief Medical Officer on the quality, suitability and effectiveness of completed investigation reports.
Develop carefully considered recommendations from serious incident investigations which will lead to organisational learning and the prevention of recurrent incidents
To ensure that events reported are analysed for trends, and issues identified in each action plan, where appropriate, are populated onto the Risk Register maintaining an effective monitoring process, whilst ensuring all elements of the risk management strategy are followed.
To co-ordinate the production of reports on patient safety/PSIRF for internal groups and reporting/assessment requirements of external bodies
We operate from three main hospitals-Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community healthcare premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.
FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.
WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.
All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.
