Job summary
The Church Street Surgery is seeking to recruit an Advanced Nurse Practitioner for a full-time position, working 37 hours per week. The ideal candidate should be an experienced nurse capable of operating within their professional boundaries to deliver comprehensive care for patients attending the practice. This includes tasks such as initial history taking, clinical assessments, diagnosis, treatment, and care evaluation. The role demands safe and informed clinical decision-making, along with expert care that encompasses both in-house consultations and home visits. The candidate should demonstrate advanced clinical decision-making and diagnostic skills suitable for general practice.
The candidate must be a registered independent prescriber with the Nursing and Midwifery Council (NMC).
Applicants who are Advanced Clinical Practitioners (ACPs) or Paramedics will also be considered, provided they have the First Contact Practitioner (FCP) qualification and are qualified prescribers.

Main duties of the job
They will workcollaboratively with the general practice team to meet the needs of patients,supporting the delivery of policy and procedures. As part of the Wyre Forest HealthPartnership, the role will be based at Church Street Surgery, but the postholder may be required to work at other WFHP sites.

Job responsibilities
Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.
Treat patients wishing to see a health care professional making any necessary referrals to other members of the primary health care team.
Ensure clinical practice is safe and effective and remains within boundaries of competence and to acknowledge limitations.
Ensure accurate and legible notes of all consultations and treatments are recorded.
Prescribe safe, effective and appropriate medication as defined by current legislative framework and considering the Worcestershire prescribing formula guidelines.
To liaise with a doctor regarding patients with complex problems who need the expertise of a general practitioner.
