This job opportunity seeks passionate and community-oriented individuals to join the Urgent Community Response (UCR) Team as Band 7 Community Matrons. The position emphasises admission avoidance through rapid response assessments and hospital-at-home interventions, providing patients with alternatives to hospital admission.
Key Responsibilities:
1. Collaborate with the Virtual Ward, Integrated Front Door, and Home-Based Intermediate Care multidisciplinary teams (MDTs) to deliver effective interventions.
2. Oversee patient care, ensuring a holistic approach to meeting the needs of patients within their homes.
3. Support from a broad team, including Consultants, GPs, Advanced Clinical Practitioners (ACPs), and the Integrated Care Service (iCares), which spans across Sandwell Community.
4. Contribute to rehabilitation, case management, and admission avoidance strategies.

Main duties of the job
Main Responsibilities:
1. Caseload Management: Maintain ongoing responsibility for a caseload of patients requiring complex chronic disease management, ensuring continuity and quality of care.
2. Cross-Sector Collaboration: Operate across both primary and secondary care services, bridging the gap between different levels of healthcare to provide comprehensive patient support.
3. Admission Avoidance: Actively work within healthcare services to facilitate admission avoidance, implementing strategies to manage patient conditions effectively at home or in the community.
4. DVT Service Involvement: Provide care and interventions within the Deep Vein Thrombosis (DVT) service, ensuring timely assessment and management of patients.
5. Non-Medical Prescribing: Utilise non-medical prescribing skills to manage and prescribe medication for patients, enhancing treatment options and ensuring timely therapeutic interventions.
6. Avoiding Unnecessary Hospital Admissions: Implement strategies to manage patient conditions effectively in the community, reducing the need for hospital admissions whenever possible.
This role requires a combination of clinical expertise, leadership skills, and a proactive approach to patient care, aimed at delivering high-quality services and supporting the evolving needs of the community.
