The Mount View Practice in Fleetwood are looking for an energetic, dedicated, caring and patient focused individual with a can-do attitude, who has the desire to make a difference to the lives of the patients in our community.
The HCA will be working under the direct supervision of our Practice Nurses and strictly in accordance with specific Practice guidelines and protocols.
The HCA will assist the Practice Clinical Team in the provision and delivery of prescribed programmes of patient care.
Ideally you will have the skills for a Primary Care environment including competency in phlebotomy and working with patients with long term condition such as diabetes.
This is a 12 month, fixed term, 30 hour per week position.
Must be able to work Mon to Fri between the hours of 0800 and 1830.
Main duties of the job
1. NHS Health Checks
2. Weight Management
3. Electrocardiogram
4. Annual reviews of long term conditions
5. Carry out urinalysis and preparation of specimens for investigation by the pathology laboratory
6. Clear and restock clinical consulting rooms
7. Ensure vaccines and cold chain storage monitoring and recording is carried out as per policies and protocols
8. Prepare and maintain environments and equipment before, during and after patient care interventions including assisting GPs during the performance of minor operations
9. Help to raise awareness of health and wellbeing and how it can be promoted
10. Act as chaperone when required
11. Home Visits
12. Where appropriate, provide support and encouragement to Patients to self-manage their conditions

Job responsibilities
The Healthcare Assistant role will work under direct supervision of the Nursing Team in accordance with specific practice guidelines and protocols.
The Healthcare Assistant will assist the Practice Clinical Team in the provision of prescribed programmes of patient care.
The Healthcare Assistant will strive to improve standards of care and be involved in identifying practice population needs, supporting the contractual, local and national requirements or specifications as required e.g GMS Contract, QOF, Quality Framework, Enhanced Services.

Qualifications
* Phlebotomy Certificate of Competency
* Full, clean driving licence & access to own vehicle for home visits
* NVQ Level 2/3 in Health & Social Care
Experience
* Experience of providing direct patient care, dealing with a wide range of people.
* IT Skills
* Experience of working with EMIS / DOCMAN
* Experience of working in Primary Care
