Job summary The post will predominantly involve working on Emmer Green ward, our dedicated orthogeriatric and hip fracture unit. There will also be an opportunity to work on Woodley ward, a 28 bedded Elderly care ward that provides multidisciplinary comprehensive geriatric assessment. It includes 6 side rooms prioritised for patients requiring isolation for infection control, receiving palliative care and elderly male patients to separate them from the rest of female ward. Around 450 Hip fractures are admitted to the Royal Berkshire NHS Foundation Trust annually. The care of Hip fractures in the unit is in line with the 'Blue book' and Department of Health's Best Practice guidelines. The care has been further enhanced by the establishment of the dedicated integrated multidisciplinary Hip fracture unit from February 2015. The Hip fracture unit has a dedicated 'closer monitoring bay' for pre and post-operative patients to enhance their care and reduce any medical complications. The Clinical data is regularly submitted clinical to the National Hip Fracture Database. Main duties of the job Immediate Responsibilities of the Appointee Work alongside the medical team to provide Comprehensive Geriatric Assessment and ongoing care to the patients on Emmer Green or alternative Elderly Care ward Attend Consultant and Registrar ward rounds, and review patients independently at other times. Communicate regularly with patients and their families or GP, to obtain collateral histories, provide medical updates and agree appropriate treatment strategies. Work alongside the multidisciplinary team, and attend relevant meetings, to help plan management, including discharges. Be able to assess and understand patients with cognitive impairment, in line with guidance provide by the Mental Capacity Act. Contribute to capacity assessments and determination of Best Interest decisions, including liaising with families and attending meetings. Clinical Objectives of the Department To provide the best medical care to diagnose the underlying medical condition and improve the patient's general health To improve patients mobility and functional independence by providing Comprehensive Geriatric Assessment (CGA) as part of the multidisciplinary team.
