It is an exciting time to join the organisations across Rotherham Place to support one of our most important areas of work: system flow. This unique role is hosted by South Yorkshire ICB on behalf of Rotherham Place and works across all health and care partners.
The role offers an incredible opportunity for someone who wants to make a tangible difference to the lives of people in Rotherham.
You will work at an operational level to identify and help resolve real-time barriers to discharge from the point of arrival in the emergency department through to the community discharge pathways and collate and analyse performance reports to manage escalations and identify trends.
As well as working with patients and their families, you will work closely with clinicians, managers, commissioners, and other stakeholders to enable more people to be cared for at home as an alternative to acute care and, if they do need hospital care, facilitating their timely discharge.
Main duties of the job
To deliver in this role, you'll need to be able to navigate between the various teams and organisations, identifying barriers to discharge and engaging with key stakeholders to resolve them in a timely way. You will need to enjoy working in a fast-paced, changing environment, utilising excellent prioritisation and organisational skills as well as soft skills to identify and negotiate solutions or escalate issues where this is not possible. You will have strong analytical skills to interpret, communicate, and report on trends and issues.
System flow is complex, and we are constantly looking to improve. You will have experience of service improvement and change management, preferably in a health and/or care environment.

Job responsibilities
You will be responsible for facilitating effective and timely whole system flow, working within and across system partner organisations. This will involve:
* Attending system flow meetings to understand and communicate day-to-day barriers to admission avoidance and discharge.
* Working with front-line health and care colleagues to identify and resolve issues, escalating those that cannot be resolved.
* Acting as the named contact for the System Command Centre, coordinating across services and facilitating out-of-area escalations.
* Coordinating and summarising performance data.
* Analysing activity and information to identify themes and trends.
* Supporting transformation work.
Person Specification
Qualifications
* Educated to a degree level or equivalent level of experience of working at a similar level in a specialist area.
* Further training or significant experience to post-graduate diploma or equivalent level in project management.
* Evidence of continued professional development.
* Programme and Project management qualification (e.g., Prince2 Practitioner; Agile Project Management Practitioner or APM qualification) or equivalent experience.
