Job summary
The purpose of this role is to actively coordinate and facilitate prompt investigations, diagnostics and clinical review, collaborating with the multidisciplinary team, thereby improving quality of care and experience for patients ensuring timely access to haematology treatment. The post holder will be responsible for the creation and maintenance of a database to ensure that patients who are entered onto the patient stratified follow up pathway are appropriately managed.
We are looking for a proactive, creative thinker who enjoys problem solving to join our Navigator team. This is a varied role that brings great job satisfaction. This position is hours per week Monday- Friday.
Main duties of the job
The post holder will work closely with the multi-disciplinary teams to ensure the efficient management of the patient stratified follow up pathway. To assist in the site specific data collection processes, diagnostic cancer pathway waiting times and national cancer data sets and input on various information systems.

Job responsibilities
1. Being responsible and accountable for their practice and behaviour under the guidance and supervision of a registered practitioner and by working as part of the oncology multidisciplinary team.
2. Working with the haematology team to provide coordination of high quality patient care through on-going telephone, face to face assessment and proactive identification for needs using basic knowledge, approved tools and procedures.
3. Collaboration and communicating with the haematology team and the wider health care teams to ensure the provision of appropriate services are available to the patient and the right time.
4. Acting as a single point of contact for patients, carers and the healthcare team. To triage enquiries and referring to the appropriate hospital department or community setting.
5. Early detection of problems and putting in place action plans to reduce unnecessary in-patient admissions
6. Documenting and monitoring all aspects of patient care coordination of service delivery.
7. Support the distribution of any patient experience surveys
8. Triage incoming calls and initiate appropriate responses according to assessment tools, protocols, individual pathways and by liaising with the haematology team
9. Make pre planned outbound telephone calls to patients to assess their needs and monitor their progress. To then liaise and update the oncology team as appropriate

11. Coordinate the necessary assessments, appointments or investigations as identified in the patients care plan
12. Support the delivery of patient information to ensure all patients/carers receive appropriate verbal/written communications in a timely manner and ensuring that patients understand the contents of written communications. This may include printing out information resources and referring to the UHNM Macmillan Cancer Support & Information Service
13. Act as the patient advocate and facilitator in order to resolve issues that may be perceived as barriers to care.
14. Contribute and coordinate the holistic needs assessment and the development of a care plan for patients with non-complex needs and monitor and review this as required with the patient.
15. Evaluate outcomes of care delivery with the registered practitioner
16. To coach patients and carers to understand the signs, symptoms or situations to be aware of that would indicate concerns to raise with a healthcare professional
17. Inform patients and carers on how to make contact with the relevant services when they feel their condition or needs have changes, including what to do out of hours
18. Effectively utilise a computer and various software applications in order to enter and maintain patient data, communicate with the multidisciplinary team and complete other IT tasks required by the post
19. Discuss support options with patients and carers sensitively and ensure that the haematology team are kept informed of outcomes
20. Liaise with ward teams prior to a patients admission regarding any support needs the patient may require
21. Promote patient involvement relating to the provision of cancer care
22. To participate and display a willingness to learn and support cancer patients and develop relevant technical competency and to keep up to date with current national and local issues and development
23. To participate in departmental audits as required under the supervision of the healthcare team and input data as requested
24. To be responsible for own learning and knowledge relevant to cancer and maintain compliance with all mandatory training as per Trust policy.
25. To participate in regular formal supervision and develop a personal development plan related to the departmental competency frameworks to promote learning and enhancing skills
26. To be responsible for managing own daily timetable of delegated tasks including time for supervision and training.
27. To access and input confidential and sensitive patient information through integrated databases in accordance with the agreed policy and procedure
28. Carry out administrative duties as required by the role.
29. Demonstrate an awareness of the limits of own practice and knowledge and when to seek appropriate support and advice
30. Assist the healthcare team to develop education and information resources for the general public and other health professionals regarding all aspects of cancer and cancer prevention
31. To maintain accurate records/documentation of enquiries while respecting the right to patient confidentiality utilising written and electronic methods iPortal, MedOnc and Somerset Cancer Registry. Any matters of a confidential nature including particular information relating to patients, their treatment and diagnosis, individual staff records must under no circumstances be divulged or made available to any unauthorised person(s).
