Specialist Physiotherapist - Community Rehabilitation
Join to apply for the Specialist Physiotherapist - Community Rehabilitation role at East Suffolk and North Essex NHS Foundation Trust.
We have been successful in securing funding to support Discharge to Assess pathways in North East Essex. This exciting development enables us to fully integrate a range of options to support discharge plans & ongoing rehabilitation from hospital, working closely with hospital teams, reablement and homecare.
We are looking for an experienced, motivated clinician to join our team. You will be a registered Physiotherapist with demonstrable expertise in goal-focused rehabilitation, supporting patients to become experts in their own care & to remain in their own homes.
It is expected that you will have excellent communication skills, enjoy working with a variety of service users of different age groups, have good self-management skills & an ability to prioritise your case load. You should ideally have experience of working in a variety of clinical settings with knowledge of the challenges of community working.
This is a Fixed Term post until 31st March 2026.
Internal applicants currently employed by ESNEFT will be offered on a secondment basis only.
Part-time hours may also be considered.
The key objectives for this post are to provide goal-focused rehabilitation, supporting patients to become experts in their own care & to remain in their own homes by providing holistic assessments & interventions for this care group.
As a Senior Physiotherapist, you will have the opportunity to undertake a leadership role in planning, co-coordinating, delivering & evaluating the rehabilitation provided and be responsible for the supervision of junior staff. You will be able to demonstrate the ability to build networks and relationships over a number of sites and different clinical teams. You will embrace innovation and integrated working and will be committed to delivering excellence in care alongside other Physiotherapists, OTs, Technical Instructors & Assistant Practitioners.
It is essential that you are a car driver with access to a vehicle for work.
We are ESNEFT and we provide hospital and community health services to almost one million people across east Suffolk & north Essex. Our dedicated staff deliver care from acute hospitals in Colchester & Ipswich, community hospitals, surgeries, community clinics & in patients’ own homes.
