We are looking to recruit a dedicated, enthusiastic, and motivated Advanced Nurse Practitioner to join our small friendly team, based in Monkwearmouth Health Centre, Sunderland.
The post holder will be a caring, compassionate and experienced nurse who, acting within their professional boundaries, will provide care for our patients including initial history taking, clinical assessment, diagnosis, treatment and evaluation ofcare. They will demonstrate safe, competent clinical decision-making and expert care, includingassessment and diagnostic skills, for patients within the practice.
The post holder will communicate andwork collaboratively with the Practice Manager and GPs, as well as our practice team, to meet the needs of patients, supporting the deliveryof policy and procedures.
Main duties of the job
To be responsible for thedelivery of a high standard of patient care as Advanced Nurse Practitioner in GeneralPractice, using advanced, autonomous clinical skills and a broad in-depththeoretical knowledge base. To manage a clinical caseload, dealing with presentingpatients' needs in a Primary Care setting. The Advanced Nurse Practitioner willneed to prioritise and triage the needs of patients accordingly making anynecessary referrals to secondary services for diagnosis and treatment.
About us
We have approximately 4700 patients and based in Monkwearmouth Health Centre. The team consists of 2 GP Partners, 3 Practice Nurses and 1 HCA together with a strong supporting Administration team. The practice works collectively with Primary Care Network practices and ARRS roles within the locality. The practice was CQC inspected in 2015 and awarded a Good rating - friendly and flexible.
Job responsibilities
Provide the choice of directaccess to an Advanced Nurse Practitioner, both in the practice, over thetelephone and via home visits for the general practice population.
Make professionallyautonomous decisions for which post holder is accountable.
Prescribe safe, effectiveand appropriate medication as defined by current legislative framework.
Provide a first point ofcontact within the Practice for patients presenting, both in the Practice andover the phone, for patients presenting with undifferentiated, undiagnosedproblems, making use of skills in history taking, physical examination, problemssolving and clinical decision making, to establish a diagnosis and caremanagement plan.
Recognise and work within own competence and professional code of conduct as regulated by the NMC.
Produce accurate, contemporaneous and complete records of patient consultation, consistentwith legislation, policies and procedures
Prioritise, organise and manage own workload in a manner that maintains and promotes quality
Deliver care according to NSF, NICE guidelines and evidence-based care
Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation.
Prioritise health problems and intervene appropriately to assist the patient in complex, urgent oremergencysituations, including initiation of effective emergency care.
Support patients to adopt health promotion strategies that promote healthy lifestyles, and applyprinciples of self-care.
Instigate necessary invasiveand non-invasive diagnostic tests or investigations and interpret findings andreports.
Clinically examine and assess patient needs from a physiological and psychological perspective,and planclinical care accordingly.
Provide safe, evidencebased, cost-effective, individualised patient care.
Refer patients directly toother services and agencies as appropriate.
Provision of a holisticservice to patients and their families, developing where appropriate anon-going plan or care and support, with an emphasis on prevention andself-care.
Contribute to thedevelopment and delivery of specialist primary care services, such as long termdisease management.
Work with the Practice teamto ensure that National Service Frameworks are being delivered.
Contribute to the practicesachievement of its quality targets to maintain the high standards of patientcare and service delivery.
Contribute to thedevelopment and set up of new patient services and participate in initiativesto improve existing patient services.
In partnership with other clinical teams, collaborate on improving the quality of health careresponding to local and national policies and initiatives as appropriate
Support and participate in shared learning across the practice and wider organisation

* Registered General Nurse MSc or equivalent
* Advanced Nurse Practitioner qualification or equivalent experience
* EXPERIENCE
* Nurse led management of minor illness and ailments
