Main duties of the job
The successful candidate will be a part of a multidisciplinary primary healthcare team, delivering primary healthcare nursing including elements of occupational health. Working alongside another B6 Practice Nurse and the Senior Medical Officer within the medical facility (MF), they will act as a focal point for Primary Health Care nursing issues within the area of practice.
We are searching for someone who can manage an individual case load, and through consultation identify, assess, and diagnose patients with complex health needs, minor injuries, minor illnesses, and specific diseases within their level of competence.
Job responsibilities
Duties and responsibilities for the role include but are not limited to:
* Providing specialist high-quality person-centred care which always considers people's safety, privacy, and dignity. Using advanced clinical knowledge that is evidence-based and cost-effective.
* Undertake preventative health programmes e.g., travel and vaccination clinics, new patients screening and Audiometry.
* Manage the care of patients with chronic illnesses e.g., Asthma, Diabetes, Coronary Heart Disease (CHD) and hypertension, ensuring continued maintenance of the Chronic Disease Register.
* Action technical nursing treatments and procedures for patients with complex/specialist healthcare needs (e.g., wound care, sutures removal, electrocardiogram (ECGs), Cytology, Audiometry and Ear Care).
* Carry out health and well-being clinics (e.g., smoking cessation, weight management, alcohol misuse) using health promotion, health education, screening, and therapeutic communication skills.
* Planning, managing and completing episodes of care by working in partnership with others, delegating and referring (as appropriate) to optimise health outcomes and resource use. Providing direct support to patients and clients.

Person Specification
Qualifications
* Professional and specialist knowledge in primary healthcare acquired through post graduate degree/diploma or actively working towards this OR can demonstrate equivalent level of competence and experience.
* Learning and Assessing in Practice Qualification or equivalent practice assessors training.
* In-date Basic Life Support (BLS), Anaphylaxis, Automated External Defibrillation (AED), Infection Prevention and Control (IPC), Safeguarding Level 3 (Children and Adult depending on population at risk) or willingness to undertake training.
* Quality improvement methodology or willingness to undertake training.
* Please provide your membership registration number.
* You are required to be registered with the relevant professional body, as detailed in the advert, prior to submitting your application.

Registrations and Memberships
* Registered Nurse on Part 1 of the Nursing and Midwifery Council (NMC) register.
* Current registration with the Nursing and Midwifery Council.
