Who You Are As a Mental Health Social Worker in a community setting, you hold a DipSW or CQSW qualification and are HCPC Registered. You have experience and knowledge of mental health work, particularly within a statutory framework, and demonstrate excellent written and verbal communication skills. You thrive in a multidisciplinary team and have experience in care management. Ideally, you're an Approved Mental Health Professional (AMHP) and possess a driving license. What the Job Involves Your role involves providing assessment, care management, and social work services as part of the Adult Mental Health team. You'll collaborate with a multidisciplinary team to assess needs and establish service eligibility under care guidelines. As a social worker, you'll manage, monitor, and review care plans, coordinate care packages, and act as a care coordinator. You'll use information management systems for record-keeping and comply with local policies while participating in safeguarding inquiries and the borough's AMHP rota. Your duties include providing practice supervision to students on placement, staying updated on legislation, policies, and participating in relevant training. You will liaise with mental health staff and other professionals across various organizations, ensuring all duties align with Council and Trust Policies.
