Your role will involve providing excellent care for our patients and be responsible for the delivery of services, working as part of our multi-disciplinary team.,
* Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects on the patients health
* Provide routine nursing care to patients as required in accordance with clinical based evidence,
* Provide vaccination and travel medicine services
* Provide wound care, ulcer /Doppler etc. to patients
* Request pathology services as necessary
* Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary / specialist care as required
* Maintain accurate clinical records
* Ensure read codes are used effectively
* Implement vaccination programmes for adults and children
* Support the clinical team with all safeguarding matters, in accordance with local and national policies
* Deliver opportunistic health promotion where appropriate
* Provide baby immunisation
* Provide cytology
* Manage emergency drug kit
Meddygfa Gyffin and Llys Meddyg Surgeries, based in Conwy, are part of the Llanfairfechan Group Practices.
Our highly skilled clinical team comprises of GPs, Advance Nurse Practitioner, Practice Nurse, HealthCare Assistant, Phlebotomist, Physiotherapist, Occupational Therapist and Pharmacy Tech. Supported by our Care Navigation and Admin teams who work across both sites in Conwy.
