This is an exciting opportunity to be part of developing a new service and shaping your role within it. As part of our services across Oxfordshire, we are implementing Primary Care Mental Health hubs to support GP practices with Mental Health assessment and support for patients in their communities.
We are looking for Integrated Mental Health Hub Clinicians who are experienced in delivering evidence-based interventions in a community setting, with a passion for partnership approaches to care for adults living with serious mental illness.
These roles need people who are innovative and want to help achieve our vision for cross-sector integrated pathways, removing existing barriers, to deliver better access to dedicated mental health support in local, non-stigmatised community settings.
This will be an exciting development opportunity as you will be part of this new service. It is a rare opportunity to help shape the new Keystone mental health teams as an integral part of the clinical team, working alongside the lead clinician and colleagues in the team.
Responsibilities:
1. Support the Clinical Lead in the development and implementation of the service and your role within it.
2. Supervision and leadership for appropriate staff identified.
3. Screen, triage, assess and coordinate referrals.
4. Provide brief support and time-limited evidence-based interventions.
5. Support patients ready to transition to primary care and develop care plans and prepare people for step down.
6. Map community resources and assets that are available for those with SMI within the specific PCN geography.
7. Support collaborative working between the full range of physical health and mental health services.
