We are looking for aCommunity Nursing Care Co-ordinator.As the CareCoordinator,youwill havetheoverall responsibility for the coordination of the regular care coordination meetings and the smooth running of integrated care within the team setting.
The key role of the care co-ordinator will be to schedule all meetings across all thegeneral practicesin their cluster, to manage the meeting agenda items, ensuring all new referrals areidentifiedand information circulated to team members in advance of the meeting. They will update the service user records contemporaneously on the identified IT systems.
• Coordinate and manage the administrative functions for the integrated caremeetings. This will include organising and adjusting meetings and tasks onbehalfof several stakeholders.
• Be the point of liaison for service users receiving care coordination andinterface with all professionals involved in service users care includingprimary, secondary, South Central Ambulance Service, community services,carersand other relevantgroups
• Coordinatethehandover with other teams tofacilitatesafeand effectivetransition of care between services to provide seamless support forpatients,theirfamiliesand carers.
• Liaisewithmedicalandnursingpersonnelinprimary,secondaryandspecialistcentresaboutpatientsandserviceprovisionrelatedmattersincludingstatutoryand voluntaryprovidersof care
• Liaise and communicate with referrers, patients, relatives, carers, and otherhealthandsocialcareprofessionalstosupportandcoordinatecare.
• Responsibility for the co-ordination of the regular care co-ordination meetingsandthesmoothrunningofintegratedcarewithintheteamsetting.
• Coordinatethenecessaryassessments,appointmentsorinvestigationstofacilitatethe provisionoftimelycare andtreatment.
• To manage and priorities workloaddailyand deal with thecompetingdemandsof theintegratedcare team

The “must haves” for this role:
1. Experience of providing confidential administrative support to a complex and varied service
1. NVQ4 or NVQ3 or recognised certificate in office administration or equivalent experience
1. Competency and experience with filing, information recording and other office management systems
1. Flexible approach to the planning and co-ordination of patient pathways to ensure maximum use of clinical time.
