The role of the FLS coordinator is integral to the Fracture Liaison Service as we develop and monitor the pathways of care for our group of patients. The postholder will have a pivotal role in enabling the Trust to deliver a high standard of care and communication throughout the fragility fracture patients pathway. Effective tracking of the pathways will also facilitate the Trust in achieving CCG Commissioning Parameters, NICE recommendations, and national standards of care for patients with fragility fracture. The postholder will have responsibility for the organization of the FLS administration activity working closely with the Fracture Liaison Specialist practitioners. You will be responsible for the organisation and tracking of a defined group of patients across all departments to which they may transverse (Radiology, Rheumatology, Outpatients BIH and MBC).These patients may need to be identified from other sources and tracked. The postholder will also provide day-to-day intelligence about progress, troubleshooting blockages in patient pathways and providing proactive information in anticipation of potential problems, escalating as necessary.
