We are delighted to advertise the opportunity for a Band 6 Occupational Therapist to join our expanding therapy team within the Sister Winifred Laver Centre. This is a sixty-bed intermediate care facility for service users requiring a period of recovery, rehabilitation, or reablement. The centre provides integrated care from Gateshead Local Authority and Gateshead Health NHS Foundation Trust and offers excellent opportunities for MDT working.
These roles are suited to an existing or emerging Band 6 who has a passion for supporting functional assessment and treatment planning to maximise independence, health, and wellbeing for our service users.
You will have excellent opportunities including service improvements and developing the role. You will be supported by the clinical MDT as well as the Occupational Therapy Department, benefiting from regular clinical and professional supervision, peer support, and access to CPD opportunities.
You will be required to work a shift pattern which includes weekend working/bank holiday rota. The post holder is required to hold a UK driving license with access to a vehicle for work purposes. You will be based at one of the Gateshead Health Locations or as directed by your line manager.

Main duties of the job
The post holder will work effectively within a multi-disciplinary/agency team attending to patients identified as requiring intervention to prevent their health and wellbeing from deteriorating and to promote and encourage rehabilitation. In addition, there will be planned activity to provide hands-on care and holistic assessment to prevent avoidable admission to hospital, and/or long-term care, and to maximise patient independence/promote rehabilitation to allow the patient to return to their own home.
This post holder will be required to ensure the fundamentals of care are provided at the point of contact and will ensure delivery of a range of appropriate interventions to meet the needs of the patients' health and wellbeing, within the community setting. Patients accessing the service will have complex needs and potentially numerous comorbidities including frailty. The post holder will support the senior team in the safe and effective delivery of patient care.
This post will contribute to the safe delivery of effective rehabilitation, working flexibly over 7 days per week.
Job responsibilities
* The post holder will assist in the delivery of occupational therapy in the SWL PIC and across a range of health and social care locations. This will involve loan working.
* To be professionally and legally responsible and accountable for all aspects of your own work including the management of patients in your care.
* Responsible to a Band 7 Occupational Therapy Team Lead.
* To work as part of the multi-disciplinary team to deliver a holistic patient-centred approach to care. To regularly liaise with MDT members including those from health, social services, and voluntary agencies to ensure an integrated and comprehensive service is provided.
* To ensure that patients are identified for prompt discharge from the service and handover to community services as required.
* Ensure that the day-to-day coordination and delivery of service is in a timely and professional manner whilst demonstrating the ability to work flexibly to effectively meet the needs of patients.
* To provide leadership for junior/unregistered staff, as agreed with senior staff.
* To support the Band 7 Team Lead Occupational Therapist in planning, coordinating, and delivering the service provided to patients and to assist in the development, planning, and progression of service provision.
* To deputise for the Band 7 Team Lead Occupational Therapist in their absence as required.
* To supervise other qualified and non-qualified staff within the team.
* To supervise Occupational Therapy students on clinical placements.
* To participate in CPD and develop skills and knowledge through participation in in-service training.
* Maintaining records as an autonomous practitioner.
* The post holder will be expected to work flexibly over 7 days.
The post will involve travel locally, and the post holder will be required to work flexibly to support service needs across Clinical Support and Screening Business Unit.
