The successful applicant will be supported to gain the competencies to provide a wide range of nursing interventions within patients' own homes. Our aim is to provide high quality, safe services that improve the health, wellbeing and independence of the people entrusted to our care, whilst also encouraging all team members to extend and develop their skills.
Working Pattern: The service operates between 08:00 and 20:00 hours, seven days a week. You will be required to work shifts between these times, but we are willing to explore candidates who would like to consider flexible working, part time, full time and possible bank hours.
Main duties of the job
* Assess, prescribe and deliver care for patients residing in the community setting to support self-management and improve quality of life.
* Provide education, support and prescribing planned care for individuals and their carers/family with long term conditions, such as diabetes and wound management.
* Support patients at the most sensitive periods of their life, including palliative care and syringe driver management.
* Work as part of a multidisciplinary team, linking with the local Primary Care Network, wider community stakeholders and your immediate team which consists of Community Nurses, Community Matrons, Physiotherapists, Occupational Therapists, Assistant Practitioners and Healthcare Assistants.
* You will be supported to maintain competencies and training in specialist skills, such as bowel care, leg ulcer management, catheters, venepuncture, IM injections etc.

Alongside a supernumerary period, with planned mandatory training and additional clinical skills courses with our Clinical Education Team as well as within the Coastal Team itself, you will be supported in your role by Senior Band 6 Nurses and an experienced Clinical Lead Nurse and Clinical Operations Manager.

Job responsibilities
* Assess, plan, implement and evaluate all aspects of patient care, and develop care plans, using clinical reasoning skills, that may need to be delivered from a range of options.
* Following a holistic assessment, devise an individualised, evidence-based care programme for each patient, modifying it as required.
* Assess patients' holistic needs, communicating complex and sensitive information to patients and carers regarding their assessment, diagnosis, prognosis and treatment plan.
* Where there are barriers to understanding, such as hearing impairment, mental capacity impairment and other difficulties in comprehension, explanations require adjustments in order to gain understanding, consent and concordance.
* Use tact and persuasive skills to gain the patients' cooperation in their care management plan.
* Liaise with GPs, Social Services, inpatient teams, other allied health care professionals and the voluntary sector to ensure identified needs are met and care coordinated appropriately.
* Be aware of assistive technology and utilise it where appropriate.
* Participate in multi-disciplinary/multi-agency meetings as appropriate, e.g. Gold Standard Framework.
* Ensure own caseload is planned and prioritised according to service targets and patient need.
* Ensure patient-held records are completed for each visit, and that SystmOne inputting is completed on a daily basis.
* Demonstrate dexterity and coordination when using specialist equipment and fine tools, advanced sensory skills, manual and mobilising skills.
* Contribute to the requisitioning of supplies and equipment through electronic ordering systems.
* Ensure informed consent is obtained prior to initiating interventions.
* Be prepared to be exposed to bodily fluids, infected material, and blood products on a daily basis, therefore must utilise universal precautions and adhere to infection control policies.

Qualifications
* Diploma or 1st level degree.
* RN Qualification.
* Current NMC registration.
* ENB 998/mentorship preparation or equivalent.
* For Preceptorship posts only: Mentorship qualification is essential for the post, but not essential at interview; however, it should be undertaken within the first year of employment.

Salary: £29,970 to £36,483 per annum pro rata.
