About the Role As a Support Practitioner, your responsibilities may include: Social Support - Helping individuals engage with their communities by planning and attending outings such as day centres, cinema, swimming, and other leisure activities. Medical Support - Administering and monitoring medication. Personal Care - Supporting wellbeing by assisting with bathing, dressing, incontinence care, and arranging healthcare appointments. Practical Support - Encouraging daily household tasks such as cleaning, laundry, ironing, cooking, and shopping to help individuals maintain their tenancy.
