Are you a motivated and experienced pharmacist looking for an exciting opportunity in a supportive and friendly environment? Lime Grove Medical Centre is seeking a dedicated Practice Pharmacist to join our team on a part-time basis.
We are looking for an in-house pharmacist to work 24 hours per week, days to be discussed. A hybrid role (in-house and remote) may be considered for the right candidate.
Main duties of the job
As our Practice Pharmacist, you will work alongside our team of GPs, nurses, and healthcare professionals to improve patient care and support safe, effective, and efficient medication management. Your role will involve:
* Resolving prescription and medication queries.
* Supporting the repeat prescription system and managing acute prescription requests.
* Conducting clinical medication reviews, especially for patients with complex polypharmacy.
* Working proactively to enhance care quality and operational efficiency.

Key Responsibilities
Medication Management
* Manage repeat prescriptions, ensuring safe and effective medicine use.
* Handle acute prescription requests and medicines reconciliation on care transitions.
* Reconcile medicines post-hospital discharge and set up systems to ensure continuity for high-risk patients.
* Develop and implement repeat prescribing policies and processes.
Patient Consultations
* Provide expert advice and support on medications.
* Collaborate with GPs and other healthcare professionals to optimize patient outcomes.
Clinical Services
* Support chronic disease management clinics.
* Suggest medications within your scope of practice, adhering to local and national guidelines.
Education and Training
* Maintain professional competence through CPD activities and clinical audits.
* Provide training for practice staff on medication-related topics.
Quality Improvement
* Contribute to achieving QOF and other performance targets.
* Implement changes from MHRA alerts and other guidelines to enhance patient safety.
Person Specification
Qualifications
* M.Pharm or BSc/B.Pharm
* Registration with the General Pharmaceutical Council
* Member of the Royal Pharmaceutical Society of Great Britain
