This will be online or over the telephone and you will be responsible for signposting patients correctly to ensure they receive the right care, at the right time, with the most appropriate healthcare professional for their individual care needs. You will also provide key support to the acute care team for patients requiring access to same day care. This is a fast-paced role where a keen eye for detail, communication skills and the ability to work as a team is essential.
