Due to retirement of our long standing Practice Nurse, Dale Medical Practice are looking for a Part time Band 5 equivalent nurse to support our existing clinical team. Salary commensurate with experience
The role is 20-25.
Practice experience is desirable.
Main duties of the job
The postholder will work within guidelines and protocols to undertake treatment room nursing duties and assist with nurse run long term condition clinics. Have change-management skills and ability to support patients to change lifestyle. Good IT and communication skills

Dale Medical Practice is a forward thinking GMS Training practice with 6500 patients. We have 3 GP Partners, 1 Salaried GP, 1 ANP and Practice Nurses, together with PCN ARRS staff we have a FT GPA and SNA and access to Pharmacist, Dietitian, Physio, Mental Health Practitioners, Social Prescriber, Health and Wellbeing Coach and support from Care Coordinators.

Job responsibilities
KEY RESPONSIBILITIES
Clinical care delivery general
Working within guidelines and protocols undertake and record:
Urinalysis and preparation of specimens for laboratory analysis
1. Temperature, pulse and respiratory rate
2. Weight, height, waist circumference and Body Mass Index
3. Measurement of legs for compression hosiery
4. ECGs, ambulatory blood pressure monitoring
5. Near patient testing blood glucose, lipids and INR
6. Doppler ultrasound measurement working with registered nurse
7. Measurement of airway function (peak expiratory flow rate)
Obtain venous blood samples from individuals for investigation.
Administer medications by injection with the appropriate legal authorisation.
Recognise health promotion opportunities and give brief, focussed lifestyle advice using motivational interviewing techniques which may include:
8. Alcohol screening
9. Smoking cessation
10. Weight management
11. Sexual health
12. Undertake initial assessment of patients presenting with wounds, working within expertise to manage independently or refer to other services as necessary.
13. Remove wound closure materials including sutures, clips and staples.
14. Take samples for the cervical screening programme according to recognised standards.
15. Administer vaccines to adults and children according to the UK immunisation schedule.
16. Provide a pre-travel health service including administration of vaccines and advice after undertaking appropriate risk assessment.
17. Administer prescribed therapies such as contraceptive injections, vitamin B12, and depo neuroleptic medications recognising side effects and contraindications.
18. Develop and follow referral pathways for patients with specialist needs.
Long term conditions
Assist nurse-run long term condition clinics:
19. Carry out clinical measurements and near patient testing for patients with diabetes, asthma and COPD in line with NICE guidance and Quality and Outcome Framework indicators which may include:
20. blood sugar monitoring
21. assessment of feet with appropriate risk stratification
22. spirometry
23. pulse oximetry
24. Peak Expiratory Flow rate
25. teaching inhaler technique
26. assessment of breathlessness.
27. Recognise signs and symptoms that may indicate undiagnosed long term conditions poor wound healing, winter cough in smokers and also factors that may indicate mental health problems and use relevant referral pathways.
Quality assurance/service support
28. Participate in quality improvement and innovations audit, significant event analysis.
29. Contribute to development of protocols and new services that aim to reduce unwarranted variation as outlined in the framework for nursing, Leading Change, Adding Value (May 2016)
30. Attend in-house meetings and contribute to development of the nursing service and where appropriate wider practice systems.
31. Participate in ongoing supervision and mentorship of unregistered health care staff, nursing students and junior nurses including mentorship for preceptor programmes and contributing to revalidation needs of nurse colleagues.
32. Attend in-house and external training events and contribute to continuing education of unregistered health care staff by identifying learning opportunities.
Personal/professional development
33. Complete statutory training/updating and mandatory training as defined by the practice.
34. Complete skills specific training and regular updating
35. Participate in an annual individual performance review maintaining a record of own personal and professional development including the requirements for revalidation with the NMC
36. Take responsibility for own development, learning and performance. Identify gaps in knowledge and skills and how further learning can enhance delivery of nursing service.
