We are looking to recruit skilled and highly motivated practitioners to join us as Band 6 Urgent Care Practitioners in the CRHTT, working closely with the acute mental health hospital and community teams to support people in a mental health crisis. CRHT are able to offer all the benefits of a hospital admission whilst remaining at home, including home visits from our experienced multi- disciplinary team.. We are committed to a focused recovery model. This post involves 7 day week 365 days per year working on a rota basis.
Main duties of the job
You will be an excellent communicator, demonstrating empathy and facilitating an inclusive and collaborative environment and assessment to those service users accessing CRHT services with a particular focus on:
1. Utilising your excellent clinical skills and judgement to assess individual needs and risks
2. Demonstrate an understanding of clinical interventions relevant to the care group to include education, care plans and supportive discharge planning,
3. Work compassionately and creatively to identify therapeutic and evidence-based outcomes and care plans for people who are experiencing a crisis in their mental health
4. Work closely with carers, families and partner agencies to facilitate and promote the safety and the well-being of service users and their families/carers.
5. work collaboratively across the urgent care services being responsible for supportive discharge assessments and participating in multi- agency meetings.

We are building a new hospital in Bexhill to replace our inpatient services at the Department of Psychiatry. This means that, if you are successful in your application, you will be asked to transfer to the new facilities when they open in 2025.

Previous experience of working in acute services or home treatment teams is desirable, although not essential. We offer excellent staff support through clinical supervision and teamwork.
This service is integral to our Urgent Care Pathway and ensures people presenting to acute mental health services receive a compassionate, person- centred assessment and timely care plan.

Essential
12. Registered Nurse with current registration [mental health/LD/Adult], appropriate to the job role
13. Completion of Mentorship Course/ENB equivalent
14. Evidence of continuous professional development
Knowledge/Experience
Essential
15. Significant post qualifying experience in the relevant care group
16. Experience of working alongside people with complex health problems in the designated care group
17. Experience of supervising staff
18. Experience of providing a range of clinical interventions to people in the designated care group with a variety of health problems
19. Experience of working consultatively with professionals
20. Experience of conducting clinical assessments including risk assessments
21. Experience of partnership working within and across various statutory and non statutory teams and agencies
22. Experience of working within the CPA process
23. Understanding of clinical governance
24. Previous experience of working in the community
