Community Mental Health Nurse - Band 6
We have a great opportunity for a full-time band 6 nurse to work in the Intensive Support Element of the Dementia & Older People's Mental Health Team based in Chichester. This post will sit within our multidisciplinary community older person's team and will work alongside our lead practitioners to deliver interventions to those people who have more complex or intensive needs. We aim to help people to remain safe and well at home thus enabling them to maintain the relationships that are important to them.
Our service is in the process of moving away from a more traditional model to adopt the follow me care principles advocated in NHS England's new Community Mental Healthcare Strategy. We are confident that this approach will help us to meet the growing demands for services whilst continuing to provide good outcomes for people.
The service operates Monday to Friday 9am-5pm.
The successful candidate will join a friendly, highly-skilled and enthusiastic multi professional team. We will take a keen interest in helping you to develop the skills important to this role and the post-holder will be encouraged to develop special interests and expertise in line with service and team objectives.
The ability to travel across sites is essential.
Main duties of the job
The role of this post is to actively engage those requiring an increased level of support with the aim of keeping people within their home and maintaining their wellbeing and independence as far as possible. We are looking for individuals who are dynamic, compassionate, good communicator who are able to demonstrate a holistic and person-centred approach to their work.
You will also need to be able to work cohesively with other services both within our Trust, and with other partner agencies. We follow a multi-disciplinary approach and within that context you will manage a caseload of older adults.

Job responsibilities
Clinical
9. To contribute to the management of a team caseload of clients with complex needs, using CPA principles, evidence based/client centred principles to assess, plan, implement and evaluate interventions, which will be delivered in the Dementia and Older Adults Mental Health Service.
10. To support/manage a team caseload and attend relevant MDT meetings
11. To regularly support and cover the duty rota
12. To provide specialist assessment and intervention as part of a multi-disciplinary approach, with specific responsibilities regarding the care pathway
13. To provide risk screening and assessment as well as risk management planning
14. To contribute to and facilitate evidence-based assessments and individual and group interventions
15. To contribute to the maintenance and development of the Service
16. To regularly share and supervise students on practice placement, and junior staff as appropriate
