Main Duties of the Job
* Manage the service provision for a defined caseload of individuals who have social care/health needs as specified in the Social Care Hub Operational Policy.
* Act as Care Manager where appropriate.
* Ensure all child protection, adult safeguarding, and Mental Capacity Act issues are dealt with according to policy.
* Respond to requests for assessments and review of need for people with social care needs, working in partnership with service users, carers, and other professionals as agreed within the multi-disciplinary team.
* Liaise closely with Primary Health Services and other agencies at the point of discharge to ensure a smooth transition.
* Participate in caseload supervision and workload management on a regular basis as required by the Social Care Hub Team manager.
* Positively address issues of discrimination and social exclusion.
* Act as mentor, supervisor, and assessor to social work students and ASYE social workers who are placed within the team.
* Participate in supervision, both personally and as a supervisor for junior staff within the requirements of Trust policy.
* Act up for the Social Care Hub Manager as required.

Job Responsibilities
The post holder will be based within the Social Care Hub and will work as part of a centralised Social Work Team. Within the Social Care Hub, social workers have a specific focus on the needs of adults of working age with mental health problems who have social care needs.
They will lead on:
* Care Act Assessments
* Requests for Tribunal Reports for unknown cases from Wotton Lawn/Charlton Lane/Recovery Units and people who are from out of county
* Reviews of individuals that Gloucestershire have placed out of county
* Reviews of accommodation cases
* Section 42 enquiries
* Provide a multi-organisational crisis contingency and relapse planning function
* Be the link for those that Gloucestershire has a Section 117 responsibility for, but reside outside Gloucestershire
* Undertake the interface social care transition work for young people moving into adult services.
* Act as Practice Educator for Social Work students and any ASYE social worker.
* Arrange the provision or purchase of social care/health services to meet the assessed needs of individuals and their carers in more complex cases.
* Enable access for individuals to direct payments/personal health budgets.
* Provide individuals and carers, in the community, with information and advice on appropriate sources of emotional, material, or financial assistance.
* Work in accordance with statutory and legal requirements offering specialist knowledge and consultation to the team on The Care Act 2014, Mental Capacity Act 2005, Mental Health Act 1983/2007, Human Rights Act, and a range of social care policy issues.
The social worker will maintain registration with Social Work England, holding to the associated values, in particular proactive implementation of anti-oppressive practice within a multi-disciplinary setting.

* Valid UK driving licence
* Social Work England Registration
* Experience of working with working age adults

* Good report writing and written skills
* Strong interpersonal skills

Salary: £35,392 to £42,618 a year (pro rata)
