An exciting opportunity has arisen for two experienced, forward-thinking, and enthusiastic clinical Pharmacists to join a new innovative specialist Hospital@Home Frailty Team.
We are in the process of developing a new multi-disciplinary team and pathways which involves the identification and management of frailty patients in their own homes. The service will include a frailty same day emergency care unit within the hospital.
These posts provide the opportunity to shape and develop the pharmacy service from the beginning.
The role will be based either at Darlington Memorial Hospital or University Hospital North Durham and the service will run Monday to Friday during core working hours. There is no weekend working or on-call commitment associated with the role.

Main duties of the job
The post holders will, as part of a multi-disciplinary team, provide expert medicines support to the Hospital@Home team.
You will be involved in setting priorities, developing guidelines and pathways, education and training around medicines, audit, governance, and medicines safety across primary and secondary care.
You will lead in the review of medication in frailty patients, developing pathways which promote appropriate prescribing and deprescribing.
There will be an opportunity to develop your clinical skills and build on your knowledge to independently review these patients as in-patients, in same day emergency care or within their own homes. You will become familiar with frailty syndromes and components of a geriatric assessment.
You will also be responsible for other members of pharmacy staff in the Hospital@Home team including pharmacists and pharmacy technicians. We also have well-established pharmacy teams in acute frailty, acute medicine, and same day emergency care who you will work closely with.

We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield, and Weardale as well as over 80 other community-based settings and providing care in patients' homes.

Person Specification
Qualifications
* Masters Degree in Pharmacy or equivalent. Completed a suitable postgraduate qualification (e.g., Clinical diploma) or demonstrated sufficient experience to meet RPS post-registration foundation competence standards.
* Evidence of CPD and education.
* Independent Prescriber or willing to work towards.
* Postgraduate Masters qualification in a relevant pharmacy subject. Postgraduate management qualification. Working towards a development portfolio, i.e., credentialing at advanced practice defined by the RPS. Teaching experience in clinical and classroom settings.
Experience
* Experience in leading and developing highly developed near-patient clinical pharmacy services. Able to work as an autonomous practitioner in all areas of pharmacy practice. Experience of developing good relationships with other health care professionals. Experience of providing directorate pharmacist services e.g., budgetary reports, drug usage reviews, development of drug protocols, specialist drug information, etc. Experience of both undertaking and supervising audit of clinical practice. Evidence of involvement in multidisciplinary service development.
* Experience of managing a team. Experience of providing basic HR support. Practice Research experience. Contribution to publications/conference presentations.

* Good oral and written communication skills. Able to build good relationships with colleagues, and influence the practice and behaviours of others in a positive way. Self-motivation and ability to motivate others. Team player.

Statutory Registration
* Registered as a Practicing Pharmacist with the GPhC.
* Membership of RPS or a relevant Clinical Pharmacy Organisation appropriate to the subject area.
Special Skills & Knowledge
* Good understanding of medicines reconciliation and how it fits into the larger function of hospital pharmacy. Ability to apply theoretical knowledge to ward/clinic based situation. Provision of complex information on medicines use, clinical pharmacy and financial issues to senior Consultants and managers. Membership of regional/national/international specialist interest groups relevant to field of practice. Demonstrable leadership, management and supervisory skills. Demonstrable teaching/tutoring skills with evidence of ability to deliver training to a wide range of healthcare staff, e.g., pharmacy, nursing, medical. Involvement in development of evidence-based clinical guidelines and protocols. Sound knowledge of MI resources. Good understanding of current pharmacy issues. Demonstrable understanding of key drivers, financial pressures and constraints within the NHS.
* Ability to demonstrate an area of expertise within a field of pharmacy practice. Evidence of active contribution to relevant specialist interest groups. Record of research and/or audit and/or scholarly publications.
