Job summary A number of exciting opportunities have become available within the role of Mental Health and Wellbeing Support Worker (MHWBSW) working across the Sunderland, and Washington Primary Care Networks. The successful post holders will work directly with adults who experience various moderate/complex mental health difficulties. The MHWBSW role requires a dynamic and flexible approach as patients will be seen within multiple locations including their GP practice, their homes and within community settings. The MHWBSW will be expected to deliver psychological interventions as well as providing a 'hands on 'approach which aims to meet the persons biopsychosocial needs. The role requires an element of autonomous working and decision making however support will be provided by the Primary Care Mental Health Practitioners (PCMHPs) working within the locality and appropriate clinical/caseload supervision will be delivered. Main duties of the job Work alongside the PCMHPs to deliver a plan of care to meet the persons biopsychosocial needs. Deliver psychological interventions which may consist of but not limited to confidence building, graded exposure, anxiety management, mood management and guided self-help. Support patients to attend other appointments such as social/physical which may be negatively impacting upon their mental health presentation. Integrate within GP practices, work alongside other agencies, and be involved in MDT/ multi agency discussions. Manage and prioritise a case load. Support the team to complete physical health checks for those patients who are registered on the Serious Mental Illness (SMI) register. To be involved in service improvement/delivery initiatives.
