We are looking to recruit an experienced practice nurse to join our clinical team, to deliver essential nursing care and support the health and wellbeing of our patients. You will join a team made up of GPs, Advanced Nurse Practitioners, Nursing Associates, Clinical Pharmacists, Social Prescribers, Mental Health Nurses, and Physiotherapists.
This is a part-time role of 22.5 hours, ideally covering Monday to Wednesday. We are a training practice and can support your ongoing development.
The successful candidate will also be required to take part in our Extended Access on Saturday mornings on a rota basis.
Main duties of the job
Provide a full range of general practice nursing care including childhood immunisations, vaccinations, and wound care (ideally trained to provide level 1 leg ulcer management).
Support health promotion and wellbeing, including national programmes such as NHS Health Checks, cervical screening, and lifestyle advice.
Support the management of long-term conditions such as asthma, diabetes, and hypertension.
Provide annual reviews for patients with learning disabilities and undertake mini-mental health assessments for patients with suspected dementia.
Provide additional treatment room activities such as ECGs and B12 injections.
Work with our practice and nursing team on the overall development of the practice and protocols relevant to the role.

Job responsibilities
To work with the nursing team to provide a comprehensive practice nursing service to include wound care, childhood immunisations, seasonal vaccination programmes, ECGs, and health promotion.
To work with the clinical team to achieve targets for national (QOF cervical screening) and local (Enhanced Commissioning Framework) initiatives.
To ensure professional responsibilities are met, supplying and administering medicines and vaccinations in accordance with Patient Group Directives (PGDs).
To ensure clinical governance standards are met, including managing clinical risk within own clinics and capability.
To contribute to the management of nursing services and resources, including ordering of stocks, maintaining cold chain, stock rotation, and monitoring of expiry dates.
To ensure accurate patient notes are recorded and by use of templates in SystmOne, enter appropriate Read codes to maximise achievement for QOF, ECF, and other Enhanced Service targets.

Qualifications
* Registered Nurse with current NMC registration
* Experience of working in General Practice
* Qualified smear taker
* Up-to-date qualification covering childhood immunisations and vaccinations
* Basic qualifications for management of long-term conditions in primary care
