We have a wonderful opportunity for a Case Manager with at least 12 months' experience to join our friendly Northern Case Management Team. We are passionate about providing a non-discriminatory service to people with acquired neurological damage, brain and spinal cord injuries, for both children and adults of all ages in a domiciliary setting. Our goal is to empower clients to develop their full potential and enrich their lifestyle through a comprehensive, client-focused service!
For this role, you will need to be a driver with the ability to travel and hold a UK professional health qualification, such as HCPC, NMC, or Social Worker registration. It would also be advantageous if you are an Advanced Member or working towards advanced membership status of the British Association of Brain Injury Case Managers (BABICM).

What you'll be working:
We can consider both Full or Part Time positions.
What you'll be doing:
* Advocating for the client’s needs (both adults and children) while building strong relationships with families, deputies, and litigation solicitors
* Producing action plans and SMART goals with specific timeframes relevant to the client
* Recruiting, selecting, and managing a team of care/support workers for clients
* Implementing an individual client-focused Support & Rehabilitation Plan with clear guidelines for the support team
* Monitoring and reviewing the Case Management and Support & Rehabilitation Plans, making amendments as needed
* Coordinating a multidisciplinary team of professionals to deliver the rehabilitation plan
* Compiling risk assessments for key activities and identifying risk management strategies to ensure safety
* Completing daily file notes of all contacts into the NCM network
What you'll have:
* At least 12 months' experience working as a Case Manager
* A UK professional health qualification (HCPC, NMC, or Social Worker registration)
* Full Driving Licence and access to a car
* Experience supporting people with neurological conditions in a community setting (adults or children)
* Thorough knowledge of CQC fundamental standards and key lines of enquiry
* Knowledge of the MCA, DoLS, and safeguarding frameworks
* Ability to address employee issues fairly and consistently
* Strong organizational, time management, and planning skills
* Flexible, professional, and positive approach to multi-site working

Case management is a collaborative, multidisciplinary process that assesses, plans, implements, coordinates, monitors, and evaluates the services needed to meet an individual’s health, social care, educational, and employment needs, aiming to restore them as close as possible to their pre-accident life.
What is a case manager?
Case managers are healthcare professionals, including Registered Nurses, Social Workers, Speech and Language Therapists, Physiotherapists, and Occupational Therapists. Their primary role is to work with clients to identify needs, develop action plans with SMART goals, and coordinate services to meet rehabilitation objectives, improving quality of life. They facilitate adjustments, recruit and support support workers, and manage care regimes.
