An exciting Senior Community Physical Health Practitioner opportunity has become available within the Harrogate and Ripon Integrated Community Team based in the centre of Harrogate.
Nursing applicants should already hold NMC registration and have successfully completed their preceptorship programme. Please note: Newly qualified Nurses or those due to qualify will be recruited via a separate process.
Candidates must have 18 months post qualifying experience including sign off of appropriate preceptorship or competency framework.
Main duties of the job
TEWV NHS Foundation Trust is offering an exciting opportunity for an innovative and dynamic individual to join our dedicated team in the provision of physical healthcare for patients within the two adult Mental Health teams.
As a Physical Health Practitioner, you will have a key role in carrying out physical health checks and have an oversight of weekly clozapine clinics and support for High Dose Antipsychotic Treatments (HDAT), as well as supporting service improvement in the area of physical health monitoring and improvement amongst our patient area. You will work closely with the MDT to provide a high-quality service to patients through support, intervention, and clinical expertise on the various aspects of physical health including: assessment, diagnosis and review, chronic illness management, management of acute presentations, and ongoing physical health monitoring. You will support evidence-based practice and have experience of working independently and autonomously, utilising clinical assessment skills and demonstrating sound clinical decision making. You will have oversight of a patient caseload while supporting the strategic role of developing physical healthcare across the area.

Essential
* Current professional registration with an approved professional body.
* Evidence of Continuing Professional Development.
* For registered nurses: MIP, FLIP, ENB 998 Teaching & Assessing in Clinical Practice or City & Guilds 730 Teaching in Adult Education. Must be achieved within agreed timescale.
* Recognised sign-off mentor, clinical educator or equivalent. Must be achieved within agreed timescale.
* Key skills in literacy, numeracy and ITQ level 2 (or equivalent).
* 18 months post qualifying experience including sign off of appropriate preceptorship or competency framework.
Desirable
* Leadership or management qualification.
* Post-graduate qualification in a relevant area.
* For registered nurses: Non-medical prescriber.
