The post holder will demonstrate detailed specialist knowledge of both type 1 and type 2 diabetes. They will act as a source of specialist advice in the nursing management of patients in their specialist area who require a high level of judgment, discretion and decision-making whilst as an inpatient/outpatient or in a setting external to the Trust. The post holder will provide education and training to patients, carers and healthcare professionals.
Responsibilities include:
1. Assess, plan, prioritize, implement, and evaluate nursing interventions to meet patients’ needs.
2. Be competent in all areas on Continuous Glucose Monitoring Systems, including fitting the sensors and group teaching to both staff and patients.
3. Download and interpret data and use different diabetes platforms available.
4. Undertake comprehensive healthcare needs assessments of patients, changing treatment plans as necessary.
5. Be responsible for assessing and recognizing emergencies in their specialty, interpreting information, and taking appropriate action while leading others to do so.
6. Provide specialist care, knowledge, and expertise, while empowering colleagues to ensure dissemination of knowledge and best practices.
7. Attend consultant-led outpatient clinics and report on cases as appropriate to the role.
8. Run nurse/midwife led clinics as appropriate to the role.
9. Provide highly specialist advice on the phone, liaising with general practitioners, district nurses, and hospital medical teams as appropriate.
10. Take a lead role in the organization and management of multi-professional patient care and support the specialist service to patients attending the hospital.
11. Contribute to and support the work of colleagues in the multi-disciplinary team.
