A fantastic and exciting opportunity has arisen within Central and North West London NHS Foundation Trust for an experienced and highly motivated Band 7 Team leader for Older Adult Home Treatment Service.
The post is a Band 7 Team Leader post – open to the professions of Mental Health Nursing, Occupational therapy and Social Workers. The role consists specialist mental health assessment of patients experiencing a crisis and additionally the leadership/ supervision of the HTS staff group.
The post is full-time, 37.5 hours per week over 7 days. We also have flexible working policies to support individuals achieve the work – life balance that their situation might require.
The Older Adult provision is a needs-led service and work with service users that are physically frail and have a mental health problem. The HTS caseloads are a mixture of service users who are experiencing crisis with long and enduring mental health issues, acute mental health issues or issues relating to memory loss. The team is supported by a good administrative system and offers an excellent work environment. The team works closely with our colleagues at The Riverside Centre at Hillingdon Hospital, The Hillingdon Hospital and have good partnership working with local non-statutory Dementia and community support services.

The services work with Older adults covering the Hillingdon borough and all parts of the services are based in Woodland Centre in Hillingdon. Hillingdon is home to a diverse population and contexts, from urban to semi-rural. Situated in North West London, it has good transport links with the rest of London and surrounding counties and is home to a wide range of social, leisure, and sporting opportunities available to people working and living in the area.

Key responsibilities
This Team Leader role will involve the full assessment of mental health care needs of patients in the community who have been referred to the HTS as they are in crisis/ prevention of admission/ supporting discharge from acute inpatient ward. The role will also gatekeep admissions to Oak Tree ward within working hours
The role will facilitate/ oversee HTS urgent response to incoming crisis calls and referrals from Admin/ Triage Lead and will fulfil the role of shift co-ordinator and facilitate incoming phone calls to HTS from patients and their support networks. The team leader will facilitate home visits or respond to need for urgent review in patients own home. They will carry their own caseload of clients and be expected to assess and monitor patients open to the HTS, acting as a resource and taking the lead in the management and delivery of services where appropriate.
This post will have a vital leadership role; acting as a role model for others, in communicating and maintaining good working relationships between the different services within OA and the further physical and mental health services. The postholder will take on the full line management responsibilities of a number of staff members within the Home Treatment Service.
The postholder will also be the nominated deputy for the Matron of Older Adult Mental Health Service and will provide managerial support to staff within the team. They will alsoensure that all services within their area are fully CQC compliant.
