Main duties of the job
We are seeking an experienced and enthusiastic Primary Care Nurse with a strong understanding of quality assurance.
The postholder will actively engage in practice / patient engagement and will play a vital role in providing high-quality care to our patients.
Clinically, the focus of the role is the delivery of evidence-based practice for patients with long-term conditions and management, and preventative nursing interventions to all patients.
Assist the practice and its staff with compliance of infection prevention and control policies and procedures.
Assure clinical safety and drive excellent patient outcomes.
Support the practice manager and clinical lead in service delivery.
Work collaboratively with the whole practice team to meet the needs of patients, supporting the delivery of policy and procedures.
About us
We are Cambs GP Network - a Federation of Practices across Cambridge and surrounding villages, with a social enterprise ethos - wholly owned by our local member practices. We hold the contract to deliver services at Maple Surgery in Bar Hill.
Maple Surgery has 4,000 patients - cared for by a supportive, stable team. The clinical team includes 2 GPs, Nurse Practitioners, and an HCA as well as a Social Prescriber, Community Matron and a Pharmacy Technician as part of Cambridge North Villages PCN. We have an excellent admin team including a clinical coder and secretary. The practice is located immediately off the westbound A14 just outside Cambridge.
This is a traditional small practice based in a village, that has the support of a Federation network.
Full or Part time hours considered.
Monthly clinical closure afternoon for training and team building.

Job responsibilities
Cambs GP Network has an exciting opportunity for a Practice Nurse to join our committed, hard-working and multi-disciplinary team of clinicians at Maple Surgery. The post holder is responsible for ensuring delivery of safe and effective nursing care to the whole practice population and accountable for nursing service delivery. The post holder will be an enthusiastic Primary Care nurse with a strong understanding of quality assurance.
This role will additionally allow the post holder to work collaboratively with the whole general practice team to meet the needs of patients, supporting the delivery of policy and procedures.
Job Responsibilities:
1. Support patients in making lifestyle modifications to improve their health outcomes and prevent disease progression.
2. Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients' health.
3. Conduct regular reviews and assessments of patients with long-term conditions, adjusting treatment plans and providing education to optimise self-management.
4. Coordinate care for patients with complex health needs, liaising with other healthcare professionals and community services, as necessary.
5. Collaborate with multidisciplinary teams to develop and implement care plans that address the holistic needs of patients.
6. Administer vaccinations and immunisations, including travel (depending on individual competency), according to national guidelines and practice protocols.
7. Support and manage health needs of women presenting with contraception and cervical screening.
8. Perform venepuncture and request pathology services as required.
9. Support the team in dealing with acute clinical requests and clinical emergencies.
10. Act as chaperone for patients as needed.
11. Support the clinical team with all safeguarding matters in accordance with local and national policies.
12. The role of the Practice Nurse is an evolving one, and it is anticipated that the post holder will be responsible and reasonably flexible in the interpretation of the above job description.
