Job summary The post holder will lead in the review of BSWICB ABI care packages, following discharge from Neurorehabilitation placement. Identifying outcomes for the assessed needs of individuals and assist in finding specialist placements for individuals with non CHC acquired brain injury Main duties of the job Coordinating and undertaking placement reviews and assessments and completion of CHC checklist for individuals with ABI. This necessitates a high level of assessment skills requiring tact, diplomacy and the ability to liaise effectively with colleagues, health care and multi-disciplinary professionals, service users, carers and family members. The timely review of all service user to ensure monitoring of quality assurance. This will ensure the appropriate level of care and funding, the accuracy of data held by the Non CHC ABI under the All Age Continuing Care Commissioning Team. The commissioning of appropriate ICB / Joint funded packages of care to meet the identified care needs of individuals.
