We are pleased to be able to offer the opportunities to join the Social Work Assessment Team at Borders General Hospital. We would welcome your application to join us as an enthusiastic, energetic and motivated Care Manager/Social Worker. You'll have experience in Adult Social Care and be part of the Social Work Assessment Team based in the Borders General Hospital.
We are a multi-disciplinary team with both Social Work and NHS working in partnership with service users, carers, family members and external agencies to facilitate discharges from an acute hospital and Intermediate care settings. You'll work closely with NHS clinicians, care providers, patients and their families to assess and facilitate arrangements for patients leaving an acute hospital setting. * Professional Social Work qualification
