The aim of the health and justice service is to focus on reducing the health inequalities for vulnerable people who enter into the criminal justice system.
The CYP outreach practitioner role is to work in an outreach service within the wider health and justice service. The outreach service will be a new service that is being developed to support vulnerable people who have entered into the criminal justice system. The service will have a focus on supporting adults and children who have been released from police custody and may have additional health needs. The focus of the team is to provide support for a period of up to 12 weeks in the community to reduce risks around reoffending and also support health vulnerabilities.
As a CYP practitioner, you will be helping to develop pathways for support for children and young people who have come through the criminal justice system. You will be developing connections with partner agencies and looking at providing support and interventions for children and young people. The role is a new development and it is a good opportunity to be part of a developing service and support the creation of new pathways of support for children and young people.
Main Duties of the Job
The main duties of the CYP practitioner will be to work closely within the outreach service and have a focus on the children and young people who are being referred on after coming into contact with the criminal justice services. This role will provide opportunities to think creatively and work in partnership with agencies in developing pathways of support.
To support the outreach service to develop a triage and referral pathway for children and young people and have oversight of this development alongside the team leader.

KEY RESPONSIBILITIES
Main Duties and Responsibilities:
1. Take responsibility for specialist assessment (within service specification) in Liaison and Diversion in conjunction with service user and carer, helping to promote self-determination, choice, and autonomy.
2. Provide written reports for Crown or Magistrates Courts or attend and give verbal evidence as required, offering considered recommendations to these bodies.
3. Participate in the setting of quality standards, including auditing, monitoring, and reviewing practice in line with current clinical guidance practice and policy.
4. Assess patients' individual care needs, agree and refer on to appropriate agencies.
5. Complete and contribute, where appropriate, to robust risk assessments and risk management plans.
6. Deliver and promote positive risk-taking, where appropriate, to improve quality of life and maximize occupational performance/independence.
7. Maintain an effective reporting system by observing and reporting verbally and in writing on patient conditions.
8. Utilize agreed outcome measures to review the effectiveness of processes, as part of Health and Justice Service.
9. Participate in the development of the service to ensure clinical governance processes are actively implemented at team level.
10. Assist in ensuring that the aims and objectives of the service (as set by the Team Leader) are fulfilled and identify factors which may inhibit these from being achieved.
The post holder will be required to work within the NMC Code of Practice and professional guidelines.
Person Specification
Qualifications
* Registered Mental Health Nurse / Learning Disability Nurse/General Nurse on NMC Register.
* Registered Social Worker.
* Post-registration experience, evidence of formal courses and training at diploma/degree level.
Experience
* Evidence of extensive post-registration experience working with clients experiencing mental health-related difficulties.
* Experience of working with clients who have difficulty in engaging with services and have complex mental health needs.
* Experience of working in a community setting and making decisions autonomously.
* Experience of assessing, planning, coordinating, and managing people with mental health problems in a community setting.
* Lived experience of the criminal justice system, care system, or vulnerabilities.
Skills & Knowledge
* Knowledge of the Mental Health NICE Guidelines.
* Knowledge of NMC / Code of Conduct.
* Understanding of the principles of the Care Programme Approach and Community Care Act.
* Demonstrate understanding of Equality and Anti-discriminatory practice.
* Skilled in the assessment of clinical risk and developing and communicating appropriate risk management plans.
* Knowledge of Mental Health Act, the Mental Capacity Act, and DOLS.
* Range of interpersonal skills (communication, listening, working collaboratively) maintaining good working relationships with service users/carers, other service providers, and agencies.
Personal Attributes
* Leadership skills, ability to motivate and influence others.
* Be able to meet the travel requirements of the community role with suitable vehicle insurance where applicable.
