JOB TITLE: Advanced Clinical Practitioner (Qualified)

Main duties of the job
The provision of clinical expertise at an advanced level in both physical and mental health. The direct provision of high quality evidence-based person-centred physical and mental health care to patients within a community pathway within the Mental Health Directorate.
Leading and communicating service users' mental and physical health status and care aims, both in computerised care records, and in written and verbal reports to medics and other staff.
The provision of expert clinical practice; confidently and autonomously assessing, diagnosing and treating a variety of physical and mental health issues.
Providing leadership in the delivery of high quality, safe and effective care to the wider multi-disciplinary team.
Facilitating learning through the development and delivery of specialist clinically focused education and training programmes.
Contributing to quality improvement programmes and providing assurances that evidence-based practices are implemented.
Working proactively in accordance with the principles of social inclusion; promoting independence and recovery.

Job responsibilities
JOB PURPOSE
Advanced clinical practice is delivered by experienced, registered health and care practitioners. It is a level of practice characterised by a high degree of autonomy and complex decision making. This is underpinned by a masters level award or equivalent that encompasses the four pillars of clinical practice, leadership and management, education and research, with demonstration of core capabilities and area specific clinical competence.
The Advanced Clinical Practitioner will therefore be required to embody the ability to manage clinical care in partnership with individuals, families and carers. This will include the analysis and synthesis of complex problems across a range of settings, enabling innovative solutions to enhance people's experience and improve outcomes.
Advanced nurse practitioners are highly experienced and educated members of the care team who are able to diagnose and treat conditions and refer onwards to appropriate specialists if needed.
Hold an appropriate Masters level award with appropriate modules in Advanced Health Assessment and in Non-Medical Prescribing.
Take responsibility for own learning and development by recognising and taking advantage of all opportunities to learn, in line with core professional competencies of the Advanced Clinical Practitioner, including full participation in appraisal and supervision, and acting as a role model for self-development.
Provide autonomous clinical assessment and treatment, demonstrating clinical expertise at an advanced level, in both physical and mental health/LD for patients within the forensic mental health/LD setting.
Contribute to the delivery of programmes of quality improvement.
Provide advanced and expert level of practice (including non-medical prescribing), leading to improved patient outcomes.
Provide clinical leadership and management leading to the provision of safe, quality, effective care.
Improve workforce knowledge and practice through the provision of ward-based education and training programmes, for RMNs/RNLDs, TNAs, Student Nurses and support staff, in both physical and mental health/LD.
Lead on improvements in the quality of care and safety through service development, evaluation and audit.

Person Specification
Qualifications
* Relevant Master level qualification as an ACP
* Professional regulated core qualification with the NMC/ HCPC
* Independent and supplementary prescribing qualification
* Mental Health Practitioner
* Qualifications in Project Management and/or Quality improvement
* Mentorship/teaching qualification
Knowledge / Skills
* Extensive knowledge and application in practice of leading MDTs, professional meetings, MHA
* A problem-solving approach to professional practice
* Demonstrate values that are complementary to those of the organisation service
Experience
* Extensive experience in a management/clinical/practitioner leadership role within mental health/LD setting
* Experience in assessing, formulating and managing risk for people with severe and enduring mental health difficulties
* Experience of developing clinical pathways and intervention plans
* High level of clinical expertise in working with people's mental health & Learning Difficulties
* Experience of service development and transformation
* Experience of working in a physical health setting
