To assess and manage patients on a Palliative Care pathway.
- To liaise closely with all health professionals in the Palliative Care pathway.
- To provide specialist advice and education in the field of Palliative Care.
- To take part in developing the Palliative Care service and clinical practice through involvement in management, teaching, education, research and audit activity., Clinical
1. Responsible for managing own caseload of patients and additional patients as required to meet service needs and provide support to Palliative Care Sister with caseload management and undertake caseload management with support where indicated and in line with competency.
2. To establish good communication with patients' GPs/consultants regarding patients' management and assistance with referrals for additional support.
3. Work as part of a multi-disciplinary Palliative Care Team, providing a holistic service to patients, carers and relatives.
4. To support the provision of palliative care, through direct and indirect intervention, to patients with complex palliative care needs,
5. To act as a source of palliative care advice and provide education and support to healthcare professionals,.
6. To work closely with all Palliative Care team members assisting in strategic planning, service delivery and contracting.
7. To participate fully in all aspects of governance, standard setting, development and implementation of policies, research and audit relating to Palliative Care.
8. To improve communication between all agencies involved in caring for patients on a Palliative Care pathway to ensure the aims and objectives of the specialist Palliative Care team are met.
9. Establish a system for effective communication between the wider care team before admission or prior to discharge from hospital, and throughout the patient's pathway.
10. Maintain comprehensive and clear records of all contacts with patients, carers and relatives.
11. Promote a culture of peer support within care teams and across the wider primary and secondary care settings.
12. To participate in the planning and implementation of business plans and service development.
13. Act as a professional role model., 1. To provide education, advice and support regarding Palliative Care to healthcare professionals, To provide education and support to patients on a Palliative Care Pathway.
2. To ensure continuing personal development through individual performance reviews, identifying education and training needs, complying with revalidation requirements.
3. To support mandatory training compliance as required.
Research, Development and Audit
1. To attend relevant study days, conferences, seminars and workshops to maximise knowledge and disseminate best practice.
2. To contribute towards conferences to raise profile and share innovative practice locally.
3. To link any training or development initiatives with established educational programmes.
4. To participate in research and audit projects, implementing and evaluating subsequent changes.
5. To act as a personal resource in an advisory and consultative role on all aspects of Palliative Care.
