The post holder will work as a key part of the multi-disciplinary team, having a clinical role with responsibility for planning, managing and coordinating the care of people with highly complex needs and long-term conditions within a defined caseload.
Main duties of the job
The post holder will use case management techniques to prevent avoidable deterioration in people's health and ultimately reducing the occurrence of unplanned hospital admissions.
The post holder will undertake all aspects of clinical duties as an autonomous practitioner with support and guidance from the Physiotherapy Professional Lead and the relevant service manager.
